Nearly Half of New Yorkers Say a $1,000 Medical Bill Would Push Them Into Debt

Nearly half of New Yorkers say they could not cover an unexpected $1,000 medical bill without going into debt, underscoring the continued strain medical costs place on household finances and the downstream implications for providers, payers, and revenue cycle operations. According to a January MetroPlusHealth Care Crunch Index survey, 46% of respondents reported they would need to borrow or incur debt to manage such an expense, while roughly one in three households said medical debt already affects their finances.

Key findings from the Care Crunch Index include:

  • Medical debt is common. Thirty-three percent of respondents reported that medical debt impacts their household.
  • Lifestyle tradeoffs are increasing. Forty-two percent said they have significantly reduced leisure or lifestyle spending due to health care costs.
  • Income matters, but pressure is widespread. Among households earning under $50,000 annually, 47% said health care costs take priority over non-essential spending. That figure rose to 60% for households earning $50,000 to $100,000 and 74% for households earning more than $100,000.
  • Racial and ethnic disparities persist. While 62% of white non-Hispanic respondents said they could manage a $1,000 medical bill without going into debt, only about one-third of Black and Hispanic respondents said the same.

The data also highlights challenges around insurance literacy that can directly affect coverage continuity and patient financial responsibility.

  • Insurance confidence is high, but enrollment awareness is not. While 79% of respondents said they understand how health insurance works, only 60% knew the timing of open enrollment.
  • Knowledge gaps skew younger and lower income. About half of Gen Z and Gen X respondents knew their open enrollment window, compared with stronger awareness among millennials and baby boomers. Familiarity was also lower among households earning under $50,000 and respondents with less formal education.
